Dentist Agreement & Liability Disclaimer
AlignersPlus – Terms of Service for Dentists

AlignersPlus is a clear aligner fabrication service that provides high-quality, custom-made aligners exclusively to licensed dentists and dental professionals. Our role is to manufacture aligners based on impressions and specifications submitted by dentists. AlignersPlus does not diagnose, treat, or provide direct patient care. The prescribing dentist is solely responsible for all clinical decisions, patient treatment plans, and case outcomes.
Responsibilities & Acknowledgments
By submitting cases to AlignersPlus, the dentist agrees to the following terms:
1. Treatment Oversight
The dentist is responsible for determining patient eligibility for clear aligner therapy and ensuring all treatment decisions align with the best interests of their patient.
2. Impression Accuracy
AlignersPlus manufactures aligners based on the impressions and treatment plans submitted by the dentist. Any inaccuracies in impressions or scans may impact treatment outcomes, and the dentist assumes responsibility for verifying submission quality.
3. Patient Communication
The prescribing dentist is solely responsible for explaining aligner therapy to the patient, discussing expectations, potential risks, and addressing any patient concerns.
4. Clinical Adjustments
AlignersPlus provides digital treatment plans, including proposed tooth movements, IPR, and attachment placement. However, the dentist must review and approve all plans before fabrication, ensuring alignment with their clinical judgment.
5. Informed Consent
The dentist is responsible for obtaining patient consent before initiating treatment. AlignersPlus is not liable for any patient dissatisfaction, complications, or unanticipated treatment results.

AlignersPlus expressly disclaims liability for any issues arising from the use of our aligners, including but not limited to:
1. Treatment Outcomes
Tooth movement varies by patient. AlignersPlus does not guarantee specific results or that aligners will achieve the anticipated outcomes provided in treatment plans.
2. Adverse Reactions
The dentist acknowledges that some patients may experience tooth sensitivity, discomfort, root resorption, gum recession, or other complications during aligner therapy. The dentist is responsible for monitoring and managing these risks.
3. Unforeseen Clinical Complications
Issues such as cavities, gum disease, periodontal complications, TMJ disorders, or occlusal changes may arise during treatment. AlignersPlus is not responsible for diagnosing or managing these conditions.
4. Post-Treatment Stability
Teeth may shift after treatment. AlignersPlus provides one retainer per case, but long-term retention planning remains the responsibility of the prescribing dentist.
5. Regulatory & Compliance Responsibility
It is the dentist's responsibility to ensure compliance with all local, state, and federal regulations governing orthodontic treatment and patient care.
Final Acknowledgment
By using AlignersPlus services, the dentist acknowledges full responsibility for patient treatment and agrees that AlignersPlus acts solely as a manufacturing provider. AlignersPlus is not liable for patient outcomes, regulatory compliance, or medical decisions made by the dentist.
