COMMISSION AGREEMENT

Between AlignersPlus and [Hygienist’s Name]

1. Parties

This Commission Agreement ("Agreement") is made and entered into between AlignersPlus, a provider of clear aligner solutions ("Company"), and [Hygienist’s Name] ("Hygienist"), collectively referred to as "the Parties".

2. Purpose

The purpose of this Agreement is to establish the terms under which the Hygienist will earn commissions by referring dental practices to AlignersPlus and encouraging the use of AlignersPlus clear aligners.

3. Commission Structure

AlignersPlus agrees to compensate the Hygienist as follows:

  • $150 per Regular Case (up to 30 aligners) submitted by a referred dental practice.
  • $50 per Express Case submitted by a referred dental practice.
  • $500 Monthly Bonus – If the total number of cases submitted from one or multiple referred practices meets or exceeds 10 cases in a calendar month, the Hygienist will receive an additional $500 bonus.

4. Eligibility for Commission

To qualify for commissions:

  1. 1. The referred dental practice must be a new AlignersPlus client or must not have submitted a case in the past 12 months.
  2. 2. The Hygienist must register their referrals with AlignersPlus before the practice submits cases.
  3. 3. Cases must be successfully processed and paid before commissions are earned.
  4. 4. If a referred practice is already working with AlignersPlus, no commission will be paid.

5. Payment Terms

  • Commissions will be calculated at the end of each calendar month and paid no later than the 15th of the following month.
  • Payments will be made via direct deposit or other agreed-upon methods.
  • The Hygienist is responsible for any applicable taxes on earned commissions.

6. Non-Exclusivity

This Agreement does not create an exclusive relationship. The Hygienist may refer other dental services, and AlignersPlus may work with multiple referrers.

7. Independent Contractor Relationship

The Hygienist is acting as an independent contractor and not an employee of AlignersPlus. Nothing in this Agreement shall be construed as creating an employment, partnership, or joint venture relationship.

8. Confidentiality

The Hygienist agrees to maintain confidentiality regarding AlignersPlus pricing, marketing materials, and proprietary information shared under this Agreement.

9. Termination

Either party may terminate this Agreement with 30 days’ written notice. AlignersPlus reserves the right to withhold unpaid commissions if termination is due to misconduct or violation of this Agreement.

10. Governing Law

This Agreement shall be governed by the laws of [State], and any disputes shall be resolved in the courts of [Jurisdiction].
